This office strives to provide quality care at a reasonable cost. Our fees are comparable to those charged by other health care specialists in this area. To help minimize the expense of billing and collection, it is our policy that payment for all office services be made as they are rendered. For your convenience, we accept personal checks, cash, MasterCard and Visa. For the patients that are under an HMO, co-pays must be paid before seeing the doctor. Prescriptions & Refills
For routine prescription refills, please contact your pharmacy. When necessary, the pharmacist will contact our office for approval. Narcotics and non-emergency refills cannot be made after 5:00 pm or on weekends. Routine drug refills are not considered emergencies and therefore should be approved during office hours. There will be no over the phone prescriptions of antibiotics or pain medication without seeing a provider. There are some medications/conditions that do require a 3 or 6 month follow up by the doctor for a refill approval.
Insurance
Your insurance carrier or HMO may cover all or a large part of your fees. We will assist you by answering your insurance questions and in most cases will bill your insurance carrier for you. Your responsibility is to keep your insurance information up to date and to provide us with an updated registration form on a yearly basis. Please present your insurance cards, and co-payment at the time of your arrival. HMO Referrals
If you need to be referred to a specialist, please call our office (5) business days prior to your specialist's visit. Due to the nature of Managed Care, you may need to make an appointment with our office to seek a first opinion and document your problem. Based on the criteria and nature of your problem, the doctor will refer or not refer you to a specialist. As a "Care Manager", the doctors at our office have contracted with your insurance company to provide all the necessary care at Family Health Care Center. Your insurance company's approval or disapproval of the referral is beyond our control.
Forms
We will gladly fill out your school, work, or medical forms once at the time of visit. After that time, a fee will apply for each form processed. Please allow 48 hours to have forms completed.
